2/28/2023 EnchantaVaughn Approved 1.Behavior: QMHP-C reviewed interventions from the previous session and educated Jahsim on positive communication skills to utilize while talking with his mother and other authority figures. QMHP-C engaged him a discussion to determine how his behavior has been since the last session. Ms. Cosby reports that he continues to struggle with his communication skills and not wanting to listen to her when he is told to do certain things in the home or in the school. 2. Behavior: Jahsim participated in the session as he did not need any redirection from QMHP-C. At times within the session he appeared to be withdrawn evident by him wanting to engage in activities with his younger sibling. 1.Intervention: QMHP-C utilized role play to illustrate how easy it is to not let others upset him. QMHP-C engaged Jahsim in a therapeutic activity to assist him with expressing his anger and frustrations towards those in the home and community setting. QMHP-C reiterated on using learned conflict resolution and problem-solving skills to handle disagreements with others. 2. Intervention: QMHP-C read a handout on consideration and allowed Jahsim to discuss what he gathered from the handout with the reoccurring causes of disagreements between peers/adults. QMHP-C demonstrated techniques for becoming an active listener to Jahsim. 1. Response: Jahsim appeared to be receptive to QMHP-C’ interventions and services that were provided to him during the session. Jahsim was presented with the emotion worksheet to identify his emotion, Jahsim shared that he did not recognize an emotion that he could identify with on the sheet and that his emotion was “fine”. Response: While reviewing interventions from the previous session, Jahsim shared that he has communicated with his mother, but that he continues to remain withdrawn. Jahsim shared that he already communicates his needs to his mother and others within the home setting, but at times he does not know how to communicate at times because they are not listening to him.
